A navigator is an individual who is set up to help consumers with questions and enrollment into available state and federal insurance programs.

Pick your location below and it will give you dates and times a navigator is available. Complete the information and you will be scheduled for insurance assistance.
If your appointment is by phone, your navigator will call you at the number you provide at the appointment time. The United Health Services Office will be in person.

Please call 574-314-5430 to get a navigation packet so we may assist you by phone. Items may be dropped off at our office from 9 a.m.–5 p.m. Mon-Thurs and 9 a.m.–2:30 p.m. Friday only.

What to have for an appointment:
- The legal names, dates of birth and Social Security numbers for your family living in your household.
- Proof of Indiana residency (name and address on official document).
- Proof of citizenship (usually ID and birth certificate/residency card OR a passport).
- Proof of income, if you have it: month of pay stubs, Social Security statements, previous year’s taxes, etc. for the whole household. If you have no income, we will help you write a letter of proof.
- If you do not have all of these items, you MUST know your name, date of birth, Indiana address and income for your family to apply.
